Please submit this questionnaire at least 48 hours prior to your session. Feel free to complete this immediately after booking your session, if you baby has not yet arrives you can leave the information regarding your baby blank for now and complete it at a later time.
Please keep in mind that the more information you provide, the more we will be able to tailor the session to your needs and put together a session that will fit your family and home perfectly so that it can be displayed on the walls of your home for years to come.
